Meet Jenny
the Nurse Advocate:
Her job description, "I answer questions for the residents, back-up information for their doctors, and act as a referral service for other agencies as needed."
Jenny is a R.N. She is also a dedicated mentor who provides education and information and helps the residents understand the community resources available to them. The programs offered consist of different exercise sessions, educational videos, and general and specialized healthcare. The Nurse Advocate also assists residents with medical equipment and apartment accessibility updates. They are educated on proper equipment use, insurance plans, emergency services, and guided through nursing home/hospice transfers.

Meet Romy
the Courtesy Shuttle Driver:
His philosophy on life, "Keep a good attitude and enjoy what you do. I love the positive attitude I get from my riders. They depend on the shuttle and look forward to using it. I’m having so much fun it’s unreal."
The purpose of this program is to provide transportation to the residents at Pinewood Apartments (Hortonville), Hillside Apartments (Seymour), and Crescentview Apartments (Kimberly). The residents at these sites are low-income, frail/elderly, and disabled people who have no cost-effective public or commercial transportation available to them. The need for this project was determined by surveys of the residents and an assessment of rural transportation options. Last year the Housing Authority received a grant for a wheel-chair accessible vehicle and by combining the efforts of multiple agencies provided a qualified part-time driver for these three sites. The rural transportation program has been a great success providing over 500 rides in 2007. Ninety percent of all rides are for medical purposes. The remaining 10% of requests are for social engagements and necessary errands.

- Priority is given to medical needs.
- Please call at least 48 hours in advance to schedule a ride.
- There is a $1.00 fee per ride.

ADA Accessible Remodels:
In 2008, NAHBRS plans to refurbish five apartments creating ADA accessible units for the tenants in need of special services. Two of the units are at Randall Court in Appleton. The complex has 100 apartments. Twenty-six units were remodeled previously.
Upon completion of the rehabilitation 28 apartments will be fully accessible to better serve the frail, disabled, and elderly tenants. The accessibility improvements to these apartments are expected to last 15 to 20 years. As a result of this project a larger portion of the LMI population will have access to affordable stable housing and be able to maintain their permanence and independence.
Objectives
1. Complement affordable housing, with a Supportive Service Matrix consisting of the Nurse Advocate Program, Courtesy Shuttle, and ADA Accessibility Projects.
2. Establish a reputation as a premier services agency for the non-profit sector.
